Submit Registration Form

MovementWorks requires that class participants complete the below registration form annually (once per season) to ensure the most up-to-date information is on file.

After you submit this form for each student/participant separately, you will be able to pay for multiple registration fees at one time, along with any tuition payments/payment plans.
